Gift Aid declaration

I am a UK taxpayer and would like Great Ormond Street Hospital Children’s Charity to reclaim tax on the donations I have made in the last four years and any future gifts I make. I understand that if I pay less Income or Capital Gains Tax than the amount claimed on all my donations in that tax year (April 6 – April 5) it is my responsibility to pay any difference. Please note that Gift Aid cannot be claimed by one individual on monies collected from multiple donors. e.g. birthday collections.

Please notify Great Ormond Street Hospital Children’s Charity by calling us on 020 3841 3841 or by emailing supporter.care@gosh.org if:

· you want to cancel this declaration
· you change your name or home address
· your tax status changes and you no longer pay sufficient income tax and/or capital gains tax

If you pay tax at the higher rate you can claim further tax relief in your self assessment tax return.
