Application for contactless device

Thank you for fundraising for GOSH Charity.

Filling out this form does not guarantee a contactless device can be provided. GOSH Charity has a limited number of devices that are shared across all internal and external events and may be unavailable at the time of your event. If we cannot provide a device for your event we will let you know in good time and offer alternative solutions.

Please note devices need a strong WiFi or data connection to be able to take contactless payments.

Thank you for fundraising for GOSH Charity. The GiveStar cashless payment device is owned and paid for by GOSH Charity. While in possession of the device you agree to:

  • Use the device to fundraise for GOSH Charity.
  • Once received, accept full responsibility for the device.
  • Report any issues with the device, including loss or damage to community.events@gosh.org
  • Return the device to Great Ormond Street Hospital Children's Charity by Royal Mail 2nd Class post within 7 days after your fundraising event has finished. This should cost around £9. Please note that as the devices contain a lithium-ion battery, you must declare this at the post office. Make sure the devices are turned off before posting, you are using strong and rigid packaging and that you use something on the inside of the parcel like bubble wrap to stop the device moving around. Address: Great Ormond Street Hospital Charity, 40 Bernard Street, London WC1N 1LE.
